Castle Partial Care Program

Population: Children & Adolescents

Location: 1000 Atlantic Avenue Camden NJ 08104 (Virtua facility)

One day remote/four days onsite

$24-$27 per hour

This position has no caseload, mostly managing referrals, scheduling, census and outreach.

 

• Provide individual and family treatment.
• Facilitate group therapy as needed.
• Acts as a liaison between referral sources, the patient and family and the program. Follow-up with referral sources, the patient and family and program.
• Complete discharge follow-up phone calls.
• Assist therapists in case management duties such as scheduling patient appointments, requesting documentation, etc.
• Assist in tracking referral and discharge statistics.
• Completes Needs Assessments for Perform Care for all intakes.

 

• Bachelor's degree in Social Work or related field.
• Experience  with medical front desk scheduling and great customer services. 
• Excellent interpersonal skills.
• Familiarity with community mental health resources
• Excellent organizational and time management skills.
• Ability to accept supervision and guidance
• Demonstrated ability to work independently.
