    Primary Care Behavioral Health Clinician

     

    Part Time position working 20 to 25 hours per week

     

    As a core member of the collaborative team within a primary care physician office the Primary Care Behavioral Health Clinician works closely with the medical provider and psychiatric consultant as well as the members of the primary care team and the patient. The Primary Care Behavioral Health Clinician is responsible for providing direct behavioral healthcare services as well as supporting and coordinating care for an assigned patient caseload and conducting psychosocial assessments for the determination of an action plan, appropriate treatment referrals, and/or community resources.

    Responsibilities

    • Provide therapy to patients at primary care offices.
    • Provide feedback about patient treatment needs and progress to referring medical professionals.
    • Coordinate care determined by assessment and action plan and facilitate community referrals. Work in conjunction with the medical provider and psychiatric consultant in addition to community resources.
    • Provide education on common behavioral health disorders, substance use disorders, and available treatment which includes treatment adherence, side effects of medications, or complications.
    • Monitor and track patient progress, changes in clinical symptoms, and treatment side effects or complications.
    • Provide brief interventions using evidence based techniques such as, motivational interviewing.
    • Participate in weekly consultation with the psychiatric consultant in addition to team meetings.
    • Document patient encounters, assessments, follow up, progress and treatment recommendations in EHR. Document in other systems as to be shared with medical providers, psychiatric consultant, other treating providers as needed.
    • Educate and develop relapse prevention and self-management plans with patients.

     

    Qualifications

    Candidate Licensure

    • Licensed Clinical Social Worker

     

    Candidate Experience

    • 2 years in behavioral healthcare provision
    • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
    • Experience in behavioral health and substance use disorders
    • Experience in chronic medical conditions

     

    Candidate Skills

    • Ability to provide therapy in a medical office setting
    • Ability to work independently
    • Ability to multi-task
    • Demonstrated ability to communicate effectively in a team setting while maintaining effective and professional relationships with patients and members of the collaborative team
    • Demonstrated time management and priority setting skills
    • Demonstrated written communication skills
    • Experience in screening and assessing for behavioral health and substance use disorders
    • Experience in developing treatment or action plans for behavioral health and substance use disorders while being patient centered
    • Experience in crisis assessment and crisis prevention planning
    • Knowledge of brief behavioral interventions such as motivational interviewing
    • Basic knowledge of psychopharmacology for common mental health disorders
    • Basic knowledge of chronic medical conditions such as, diabetes
